Evaluation
Upon entering a Endeavor Therapy program (i.e. Pulmonary
Rehabilitation, Physical Therapy) a licensed social worker will begin
by assessing the mood/anxiety level of the patient. If the
patient shows fairly significant signs of depression and such, the
social worker will help to resolve these issues. The social worker
will also develop communication between the referring physician and
the patient. The social worker also serves as a community resource
person to help set people up with the appropriate individuals who can
help with their needs be it transportation, meals on wheels,
financial, etc.
